Welcome to Mears Athletics!

We are all excited to welcome your student to Mears Athletics for the 2024-2025 school year! If your student is an incoming 6-7^th grader, or a new to Mears 8^th grader, here are some important things to remember regarding forms.

In elementary school, students often only need to submit a signed permission slip to participate in any ASD afterschool activities such as running club or NYO, so your 7^th grader is probably used to turning in a single form and being ready to participate. In middle school there is added risk in participation, so more documentation is required, and the rules are clearly defined. In high school the rules for participation will be even more stringent, and we are tasked with preparing them for that next step. This is important because high school athletics can provide scholarship opportunities, and proper documentation ensures that the process is fair to all students. I have provided the links below to all forms needed for middle school sports activities.

The following is a list of everything needed to register for sports activities:

1. A valid sports physical is required for participation in middle school athletics. This form must be completed by your child’s physician, or other qualified medical provider. Without this form, I cannot add your child to any roster. This is a separate document from any medical forms you submitted at registration. It will clearly state the date of exam, cleared/approved for sports or activities, signed by the medical provider, and will be stamped by the medical office.  Sports physicals are valid for 18 months from the day of exam, and must be valid throughout the activity they are participating in.

2. A new Middle School Activity Form is required for each sport your student participates in. It is important for our coaches to have updated agreements from you and your students, so you will need to authorize a new form every time.

The COVID-19 Waiver is no longer required.

STUDENTS WILL NOT BE ADDED TO THE ROSTER UNTIL ALL REQUIRED FORMS HAVE BEEN SUBMITTED! SORRY, WE WILL NOT SAVE A SPOT FOR A STUDENT WITH INCOMPLETE FORMS!
